Care Co-ordinator Phlebotomist (BACHS, Clifton and Meadows, and City South)

NCGPA are looking for an enthusiastic, confident, flexible, and highly organised person to work within the Primary Care Network (PCN) Team. This is a new and multifaceted role supporting the PCN with the delivery of the Direct Enhanced Service Contract (PCN DES), through the provision as a PCN Care Co-ordinator Phlebotomist.

To collect blood samples by Venepuncture as requested by clinicians for testing in the Pathology laboratory, this will be either in the practice or on home visit. You will work closely and in partnership with the Social Prescribing Link Worker(s) or social prescribing service provider and Health and Wellbeing Coach(es). You will also support the Primary Care Network to maintain and develop the service in line with Primary Care Network objectives.

Service Support Officer (Receptionist)

We are looking to appoint a part time Receptionist (Service Support Officer) to join our Service Support Team. As part of this team, your core responsibilities will be front line reception duties and administrative tasks.

The service operates on a hub and spoke model. The Hub sessions will be based at the Angel Row premises and the spoke sessions will be based in the PCN localities. You will also be expected to provide bank cover, bank holidays and weekend cover when needed.

Health & Wellbeing Coach

To work alongside the multi-disciplinary PCN practice teams to effectively plan and implement a high quality Health and Wellbeing Coaching service as part of the broader PCN primary care team. The post holder will work autonomously and support patients of all ages and their carers to manage their physical and mental health and wellbeing, build community resilience and make informed decisions and choices when their health changes. The individual will deliver a universal and proactive offer of support to people with long-term physical and mental health conditions to build knowledge, skills and confidence to live well with their health condition. They will also deliver intensive and integrated approaches to empower people with more complex needs, including those living with multi-morbidity, to experience coordinated care that supports them to live well, helps reduce the risk of becoming frail, and minimises the burden on treatment. Additionally, provide support to further the PCN’s goals to improve and maintain staff wellbeing.
