Social prescribing starts with a conversation. It may be a conversation with a GP or with someone else from the practice team. From 1st January 2020 patients can be referred to Social Prescribing Link Workers.
Link Workers support patients to unpick issues they may be facing. Putting them in touch with people and activities that provide practical and emotional support, enabling the patients to take greater control of their health and wellbeing.
Studies show that people who are supported by a Link Worker feel better, and get better, faster than those treated with medicine alone. And because it works, it’s happening more and more – including here in Nottingham City.
Nottingham City GP Alliance is the lead employer for the team of Social Prescribing Link Workers in Nottingham City. The team are based directly in the eight Primary Care Networks (PCN’s) in Nottingham City. Focusing support in the area local to patients ensures patients access what they need close to home.
