Personalised Care Development Day

On 4 June, we held our first Personalised Care Development Day for PCN staff across the city. This day provided an opportunity to share experiences from across PCNs, make connections with partners, and celebrate the successes they’ve had.

Our PCN personalised care staff do an incredible job in the community, creating unique connections with patients to help them live happier and healthier lives. Our Health & Wellbeing Coaches, Social Prescribing Link Workers, and Care Co-ordinators go the extra mile for patients, helping those experiencing social isolation and complex needs get back on their feet.

This development day was designed to help our teams make connections with others and take a wider look at the changing face of healthcare and how the work of PCNs fit into this structure. This was delivered through a range of activities, talks, and opportunities to network with colleagues and local partners.

The event was co-designed by Marcella Tarantini (Social Prescribing Link Worker) and Sarah McCunney (Care Co-ordinator) as representatives of their roles, providing input on what content would be most relevant and useful to those attending.

NCGPA Head of PCN Workforce Ankish Patel hosted the event, opening with his personal story as the son of Indian migrants growing up in Manchester. His story shone a light on the importance of finding community, and how the small actions of others can make the world of difference to people who feeling disconnected from society.

As well as NCGPA employees, we were delighted to be joined by representatives from local healthcare organisation in the city. They were all given chance to network with our teams to explain what they do, and the referral pathways into their services. These included:

With patient demand increasing and becoming more complex, joining up connections between organisations is vital for improving health outcomes in the Nottingham. Increasing emphasis on prevention not only improves lives and reduces suffering, but is also a vital means of reducing pressures and costs for primary and secondary care institutions.

The day concluded with an awards ceremony for outstanding individuals who have gone the extra mile for patients. The winners were:

There could have been so many more, with the managers have a difficult time narrowing it down. Congratulations to all the winners!

We would like to thank everybody that attended this first event and provided such positive feedback. We hope this will become a recurring event and will help those who attended continue to provide even better care, support, and advice for their patients.
